General dentist · Wichita Falls, Texas
Urbina Dental is one dentist's office on Kell Boulevard. We file claims for you with PPO dental plans. If your coverage has ended, or you never had any, we have a yearly price for routine visits, with its terms beside it, and you see any other fee in writing first.

Insurance first
Plan types, not a carrier list. Before your visit our front desk checks your plan's benefits and whether we are in its network, and tells you what the plan says it pays.
You pay the share your plan doesn't. Whether we are in your plan's network depends on the plan, so we check first.
These plans assign you to a dentist. We can give you a written estimate.
Original Medicare doesn't cover routine dental care. Some Advantage plans do, and we check yours.
We aren't enrolled. We can give you a written estimate.
Pay per visit with a written estimate, or read the terms on the card below.
Membership, second
The membership covers routine visits for one yearly price. It is not insurance, and it isn't used on a visit billed to insurance. The prices are examples the practice replaces with its own.
Yearly fee: ask the office
Routine care for one adult, paid once a year.
Not insurance. Not used on a visit billed to insurance.
Yearly fee: ask the office
Routine care for one child, paid once a year.
Not insurance. Not used on a visit billed to insurance.
Example prices. The practice sets its own and what a membership includes. Treatment the membership doesn't list is quoted in writing before we start.
Membership or insurance

The dentist
One dentist, so you see the same person each visit.
Dr. Urbina opened the office for families whose dental coverage comes and goes with a job: a plan that ends, a yearly limit that runs out, or none at all. She explains the options, what your plan says it will pay and what is left for you, and she leaves the decision with you.

We file claims with PPO and indemnity dental plans. We aren't in network with DHMO plans, and we don't take Medicaid or CHIP. Network status depends on your plan, so we check it before your visit.
You pay the fee for any treatment past the limit, and you see that fee in writing before we start. If your coverage has ended, routine visits have a yearly price on the card above.
Yes. You get one before any treatment, with what your plan says it pays, and nothing starts until you say yes.
Send a request and Robin will call or text you back with a time. We check your plan's benefits before you come. Please leave out health details; for anything urgent, call us.
