What we ask for on this website

The contact form asks for your name, email, phone number, how and when to reach you, whether you are a new patient, and a short message. We use it to reply to you about a visit, and for nothing else. Please don't put medical details or insurance numbers in the form.

Your health information

We collect your health history on our private patient portal, not by email or through this website. We keep your records private and share them only as the law allows.

Your choices

You can ask us what we have about you, ask us to correct it or ask us to stop contacting you. Call (940) 555-0100 or write to frontdesk@urbinadental.example.

Changes

If we change this page, the new version appears here. The practice's owner replaces this sample text with its own policy.
